Master Class Monday

With my class payment, I agree, when participating in any classes at The Performing Arts Center by either me or my child, is voluntary and at my own risk. Further, I jointly and severally hereby forever release, discharge and acquit, The Performing Arts Center, HBJN Inc., and all of their employees, choreographers, teachers and agents from any and all claims for damages or injuries of any kind, nature or description resulting from any rehearsals, classes, private instruction, rentals, workshops, or any other type of activity or event at The Performing Arts Center.

 This expressly includes, but is not limited to, any injury or damage caused by or resulting from the negligence of The Performing Arts Center, HBJN Inc., employees, teachers, agents, etc. I acknowledge the fact that certain types of injuries are common and inherent in dance related activities including all “B-Boy” or “B-Girl” Hip-Hop styles as well as any type of partnering, acrobatic movement, pointe, heel and aerial work.  This release includes but is not limited to those types of injuries.

I  understand that any student may decline to participate in any portion of a class activity.  Additionally, each student (or their parent or guardian) has the obligation to inform the instructor of any physical limitations. If there are any doubts regarding your physical abilities, please consult a physician prior to participation. Further, I acknowledge and understand that Master Class Monday is an adult, professional level class for artists 16 years old and, as such, may contain adult language, references and concepts. Anyone under the age of 18 years old participating in Master Class Monday can only do so with the consent of the teacher.  Additionally, a parent or guardian must be present observing the class at all times.
